Value-based care contracts pay for completed care episodes. A completed care episode requires that the referral converts. The referral coordination layer is VBC infrastructure, whether or not anyone has told you it is.

The executive conversation about value-based care in 2026 is dominated by analytics platforms, population health software, and risk stratification tools. That conversation is important, and it's incomplete.

Underneath the analytics is a workflow layer where the actual care episodes either complete or don't. A patient is identified as having an open care gap or a specialist need. A referral is placed. And then one of three things happens. The patient is seen. The patient is not seen. Or the organization has no idea what happened because the referral wasn't tracked.

Only the first outcome produces the quality scores, the shared savings, and the risk-adjusted revenue that value-based contracts pay for. The other two outcomes produce cost without revenue. Why does referral completion matter for value-based care?

Under fee-for-service, a lost referral is a lost appointment. The revenue impact is the visit fee. Under value-based care, a lost referral has three compounding effects.

First, the quality score hit. HEDIS measures, STARS ratings, and ACO quality benchmarks all depend on documented completion of recommended care. A referral that doesn't convert to a completed visit doesn't count toward the numerator. Second, the total cost of care hit. VBC contracts track the total cost of care for attributed patients. Referrals that leak out of network drive higher cost for services that would have cost less in-network. Referrals that never complete but still result in ED visits downstream drive even higher cost because the patient eventually shows up in the expensive pathway.

Third, the shared savings hit. For ACOs and organizations in upside or two-sided risk contracts, avoided cost is revenue. A completed specialist consult that prevents an ED visit, an avoided readmission, or a caught-early diagnosis all produce savings that get distributed back to the participating organization. Referrals that don't complete don't produce those savings.

Put these three together and the math shifts. Under fee-for-service, a lost referral is a one-time revenue miss. Under VBC, the same lost referral compounds into quality hit, cost hit, and savings hit simultaneously. The per-referral value of completion under risk-bearing contracts is meaningfully higher than under fee-for-service. This is the point most operational teams have not yet caught up to.

How broken is referral coordination under most VBC contracts?

Industry benchmarks across multiple studies put referral completion rates in the 40 to 60 percent range for practices running manual or semi-automated referral workflows. That is not an outlier number. That is the baseline.

MGMA research has documented that 45 percent of faxed referrals never result in a scheduled appointment. Health Affairs analyses have shown that 55 to 65 percent of potential in-network referrals leak even when in-network options are available. For behavioral health specifically, only 26 percent of referrals in a large EMR study had a subsequent encounter scheduled at all. When you layer those numbers onto a value-based contract, the revenue leakage is substantial. An ACO with 20,000 attributed lives generating roughly 30,000 specialty referrals a year, running at 50 percent completion, has 15,000 referrals that are not producing quality scores, not avoiding downstream cost, and not generating shared savings. The gap between the analytics layer and the coordination layer is where this happens. The analytics knows the patient needs a mammogram. What does a VBC-optimized referral workflow look like end to end?

The target state is a workflow where every referral, once placed, converts into a tracked care episode with closure metrics visible to both the referring and receiving providers. Here's what that looks like on the ground.

Referral trigger. The PCP or care gap file identifies a patient needing specialty care or a covered preventive service. The trigger can come from an EHR order, a payer gap file, or a population health stratification output.

Patient outreach. Within minutes of the trigger, the system contacts the patient through their preferred channel (SMS first, voice AI second, email as backup) in their preferred language. The message is specific, includes a self-scheduling link, and follows up on a cadence calibrated to the patient's historical response pattern.

Scheduling. The patient self-schedules through the link, or voice AI handles the booking in a conversational call. The appointment is written directly into the EHR calendar, with appointment type, provider, and insurance all pre-validated.

At every stage, exceptions get routed to human coordinators with full context. The exceptions are the 15 to 20 percent where judgment matters. The other 80 percent runs through the system without human touch.

Why does the CMS TEAM Model amplify this?

The CMS Transforming Episode Accountability Model (TEAM) goes live in January 2026, mandatory for 748-plus hospitals. TEAM applies episode-based payment to five high-volume surgical episodes (CABG, lower extremity joint replacement, major bowel procedures, surgical hip and femur fracture treatment, spinal fusion). This framing fits best for: ACOs and organizations in two-sided risk contracts where the financial exposure to incomplete care is direct, primary care groups managing Medicare Advantage populations where STARS ratings drive a material percent of revenue, FQHCs with UDS quality reporting requirements and MCO quality bonus programs, health systems preparing for TEAM or other episode-based models, and multi-specialty PE-backed groups where referrals between specialties are the norm rather than the exception.

This framing is less ideal for: organizations still substantially in fee-for-service without meaningful VBC revenue exposure (the operational case for referral completion is still strong, but the VBC-specific lens is secondary), single-specialty practices that primarily receive referrals without originating them (the benefit accrues asymmetrically to originators), practices where the specialist network is so constrained that coordination automation doesn't solve the underlying access problem.
